Please clear your craft items by emailing mauimatsuri@gmail.com before applying for a Craft Booth Reservation. If you are a returning crafter and your items are the same, you may apply without clearing your items.

Please note the following:
1. Submitting Craft Booth Reservation form, certificate of insurance or confirmation for the vendor insurance program, and applicable fees, as early as possible, will secure your booth. Booth fees are not refundable.

2. Participants are responsible for providing their own tables and chairs.

3. Booth space is limited to an area of 10’ X 10’.

4. Booth location is assigned.

5. Vendors are encouraged to use “green practices” such as using biodegradable bags, etc.

INSURANCE REQUIREMENT:
All craft vendors will be REQUIRED to meet insurance requirements.

1) If you have an existing Commercial General Liability Insurance policy, please furnish a certificate of insurance naming Japanese Cultural Society of Maui and Maui Taiko as Additional Insured. Certificate Holder shall be Japanese Cultural Society of Maui. Minimum coverage requirements for Commercial General Liability Insurance:
$1,000,000 per Occurrence
$1,000,000 Personal & Advertising Injury
$1,000,000 Products – Completed Operations Aggregate
$1,000,000 General Aggregate
$100,000 Damage to Rented Property (Each Occurrence)
$5,000 Medical Expense (Any One Person)

2) If you do not have an existing policy, you will be required to purchase the one-day special policy under a Vendor Insurance Program for a cost of $55. Email mauimatsuri@gmail.com for further instructions. After you apply for the program, you will get a confirmation email which you will attach to your Craft Booth Reservation form.

Completed craft booth reservation form, certificate of insurance or confirmation for the vendor insurance program, and applicable fees must be submitted by deadline on the form. All required paperwork must be attached to reservation form or your application will be denied!
